Becoming a patient at Henderson Primary Care
We're open and accepting new patients. Here's how the process works.

How to join
- Join our patient list. Use the Request a Call Back form (name, phone, best time to call only, no medical details, please).
- We'll call you back. A team member will reach out during your preferred time window to confirm your insurance and answer any questions.
- We'll schedule your first visit. Most new patients are seen for an establish-care visit either in person or via telehealth.
- You'll get patient portal access. Forms, history, medications, and pharmacy preferences are collected through the portal before your first visit.
What to bring to your first visit
- A photo ID
- A complete list of current medications and supplements (dose and frequency)
- The name and contact info of any specialists you see
- Recent labs or imaging results, if you have them
- A list of questions or concerns you'd like to discuss
Records transfer
We can request your medical records from your prior provider once you're an established patient, there's a release form in your portal. If you have records already, you're welcome to upload them.
Establish-care visit, what to expect
Your first visit is longer than a routine appointment. We'll go through your medical and family history, review medications, talk about screenings you may be due for, and set goals together. You'll leave with a plan and a follow-up schedule.
