Abigail Leonoras is a board-certified Adult-Gerontology Nurse Practitioner (AGNP-C), a certification specifically in caring for adults as they age, and she has spent more than fifteen years in healthcare, including nephrology, home health, hospice, dialysis, and internal medicine. That is the exact terrain older patients live on.

  • Medication review. Bring every bottle, including supplements. We look for interactions, duplicates, and prescriptions that no longer earn their place.
  • Multiple conditions at once. Blood pressure, diabetes, cholesterol, kidney function, thyroid, and heart health managed together instead of one organ at a time.
  • Memory and thinking concerns. Evaluation, screening for reversible causes, and referral when a specialist evaluation is appropriate.
  • Falls, balance, and mobility. Fall-risk review, gait and strength discussion, and the medications that quietly contribute to unsteadiness.
  • Mood, sleep, and isolation. Depression and anxiety look different later in life and are frequently missed.
  • Coordination with your specialists. Cardiology, nephrology, endocrinology, and others, with someone keeping the overall picture in view.

Most of our older patients are managing more than one thing at once. We handle blood pressure, type 2 diabetes, and the broader work of chronic disease management in the same relationship, with labs, follow-up intervals, and medication adjustments planned around how you actually live rather than a textbook schedule.

Getting here, and getting seen

Our office at 9315 W Sunset Rd, Ste 101A in southwest Las Vegas has free surface parking and a ground-floor suite with no stairs, which genuinely matters when mobility is a factor. We serve older adults across Las Vegas, Henderson, Green Valley, Summerlin, and the wider valley, and a Henderson office is opening soon. Routine follow-ups can often be done by telehealth so you are not making the drive for a five-minute check-in.

What makes an AGNP-C different for older adults?

Abigail Leonoras is a board-certified Adult-Gerontology Nurse Practitioner, which means her certification is specifically in the care of adults through older age. Her background includes nephrology, home health, hospice, dialysis, and long-term acute care, all common ground for older patients in Las Vegas and Henderson.

Do you accept Medicare?

Yes. Henderson Primary Care is in-network with Medicare, Aetna, and UnitedHealthcare, with Cigna coming soon. Your annual Medicare Wellness Visit is a separate covered appointment, described on our Medicare wellness visits page.

Can you review all of my medications?

Yes. Bring every bottle, including supplements and anything prescribed by a specialist. Reviewing the whole list together is one of the most valuable things we do at a geriatric primary care visit, because interactions and duplicate prescriptions are common when several doctors are involved.

I am worried about my memory, or my parent's memory. Can you help?

We can evaluate memory and thinking concerns, look for reversible causes such as medications, thyroid problems, or vitamin deficiency, and refer on when a specialist evaluation is appropriate. Bring a family member who sees the day to day if you can.

Can a family member or caregiver come to the visit?

Absolutely, and we encourage it. Caregivers often notice things the patient does not mention, and they are usually the ones managing the pill organizer and the calendar.

What if I cannot get to the office easily?

Many follow-up visits, medication check-ins, and results reviews can be handled by telehealth anywhere in Nevada, which saves a trip across the valley.
